Save time at your first appointment by filling out your paperwork online! Simply follow the four steps below and you’ll be registered and ready to go when you get to the office. All of our forms are HIPAA-compliant to ensure that your information is secure.
Health & Safety Guidelines
As we move past the pandemic, we want to inform you of what we’re doing in preparation for our continued commitment to your safety. As you are aware, infection control has always been a top priority for our practice, and our commitment to it is unwavering. Our infection control processes are developed so that—when you receive exceptional care in our office—it is in a clean, safe, and comfortable environment.
Bolton Family Dental has always followed and will continue to follow the recommendations provided by the Centers for Disease Control (CDC), American Dental Association (ADA), and Occupational Safety and Health Administration (OSHA). We have also added additional layers of protection, as our goal is to exceed all recommendations and provide you with even greater peace of mind.
We will continue to follow the activities of these agencies to ensure that we are up to date on any new rulings or guidance that may be issued. Below are a few of the safety protocols that you will notice at your next appointment:
The Day Before Your Appointment
Please call the office if you have experienced cold or flu-like symptoms within the last 7 days.
Practice Protocols
Magazines and books have been removed from the waiting room. We have added specialized air filters throughout the office. Our entire staff will be wearing increased PPE.
The front office has been retrofitted with plexiglass to protect staff and patients during office interactions. Extra scheduled deep cleaning of the office will occur several times throughout the day. Our doctors will continue to address as many of your dental concerns as possible by phone.
