New Patient Paperwork
Save time at your first appointment by filling out your paperwork online! Simply follow the four steps below and you’ll be registered and ready to go when you get to the office. All of our forms are HIPAA-compliant to ensure that your information is secure.
COVID Guidelines
At Bolton Family Dental, we have several protocols and practices in place to ensure that we can continue to offer you the highest quality care during the pandemic. Our goal is to exceed all guidelines and provide you with even greater peace of mind, allowing you to enjoy exceptional dental care in a clean, safe, and comfortable environment.
Below are a few of the safety protocols that you will notice at your next appointment:
The Day Before Your Appointment
24 hours prior to your visit, our staff will be in communication with you to do a pre-screening questionnaire.
On the Day of Your Appointment
Please remain in your car when you get to the office and call to let us know you’ve arrived. A staff member will ask you similar questions to the day before. We will be providing hand sanitizer for you to use upon entering. Per CDC healthcare guidelines, we require that all patients wear a mask to their appointment, and guests/companions will be asked to wait outside the office unless the patient is a minor, or if assistance is needed.
When Making an Appointment
Appointments will be managed to allow for social distancing in the waiting room and hallways. The regulations set forth by the governor have required that we reduce the volume of people being treated in a given day for the unforeseeable future. This might mean that you are offered fewer options when scheduling your appointment.
