Payment Options

Payment and copays are due at the time of service. For your convenience, we accept Mastercard, Visa, cash, and checks. We also accept HSA (Health Savings Account) and FSA (Flexible Spending Account) cards.

Insurance

At Bolton Family Dental, we accept a wide variety of dental plans, including most plans through Blue Cross Blue Shield and Delta Dental. That said, insurance companies offer more plans than ever, and coverage can vary widely from plan to plan. We strongly recommend that you check with your insurance company to ensure that our practice is covered under your plan.

CONSIDERATIONS

  • Not all dental procedures are covered by benefits in all insurance contracts. We encourage you to contact your insurance carrier directly for an explanation of your individual benefits.
  • Your insurance is a contract between you and the insurance company. We are not party to that contract and cannot guarantee what your insurance will pay. We do our best to estimate your coverage, but it is still an estimate.
  • While our office will file your insurance claim for you, final responsibility for our fee still rests with the person responsible for the account (the patient, parent, or guardian).

No-Show & Late Cancellation Fees

24-hour advance notice is required when canceling an appointment. If you do not provide 24 hours advance notice, you will be charged a fee of $50 which will be due before your next appointment.

In our desire to be effective and fair to all of our patients, we have a clear policy for cancellations, no-shows, and late arrivals. We suggest that you familiarize yourself with our full cancellation policy in order to avoid having to pay for a missed appointment!
